What it is
OpenMedicine listens to an in-office visit through the microphone, writes a live clinical note and assessment & plan, and can copy the note or write it into the Healthie chart.
Signing in
Each doctor logs in with their email + password on the home screen (Log in).
Admins can instead use the shared access code (kept as a fallback during the pilot).
Forgot a password? An admin resets it — there is no email reset link yet.
Starting a visit
On the home screen choose Start an in-office consultation and allow microphone access.
It listens through your computer/phone's default microphone and builds a live, speaker-separated transcript. There is no in-app mic picker yet — change the default mic in your operating system's sound settings if needed.
Recording never stops on its own. It only stops when you press Pause recording or Back.
Pause & microphone
Pause recording mutes the mic; press again to resume.
Only one app can use the mic at a time. Running another dictation app (e.g. Wispr Flow) during a visit will fight for the mic.
If the mic is taken, the connection drops, or you switch browser tabs, the app auto-reconnects and resumes when the mic is free. You can also press Resume mic to force it.
The three panes
Left — Live Patient Context: the structured patient context, the live transcript, and detected actions.
Middle — Interpretation + Working Clinical Note: today's focus, key findings, a top differential, must-not-miss concerns, and suggested questions/exams; below it the live note (chief complaint, HPI, PMH/PSH, family & social history, medications, allergies, review of systems, exam). Both update live on their own.
Right — Assessment & Plan + Final Note.
Interpretation
Differentials are numbered and the leading ones are shown in bold, with a red MUST NOT MISS flag for dangerous-but-less-likely diagnoses.
Press Find evidence on an item to pull supporting references, or Update reasoning to re-run it against the latest dialogue.
Working clinical note & Assessment & Plan
The Working Clinical Note drafts itself during the visit; press Copy on it to grab the plain-text note.
Press Assemble Assessment & Plan for a problem-based A&P. Each problem has a one-line summary, a short assessment, and proposed actions as checkboxes. Uncheck actions or remove a problem; press Copy to grab it.
The A&P is a snapshot — press Assemble again any time to refresh it with new dialogue.
Final Note & sending to the chart
Press Assemble Final Note to combine the note with the approved problems and checked actions into an editable note (#Problem headings, [] action items). Assemble it after the A&P — it will warn you if the A&P isn't ready yet.
Pick the note type before assembling: H&P gives a comprehensive initial note (full history, exam, labs/imaging, and A&P); Progress Note gives a focused interval/SOAP note (brief interval history + current status + updated A&P). Assemble again after switching to reformat. Use Shorten / Expand / certainty to adjust, edit freely, then Copy Final Note to paste into Epic, or Send to chart to write it into Healthie as a draft you then sign (the type also picks the Healthie form).
Pre-charting
Ask the assistant to “prepare the pre-chart for <patient>”; it pulls the chart from Healthie and shows a short pre-visit summary.
Phone ↔ computer handoff
Start a visit on the phone — a 3-digit Session ID appears at the top and is saved with the visit.
On a computer, sign in with the same account, choose Open a phone session, type the Session ID, and the saved note opens to review, assemble, and copy.
Visits auto-save every ~45 seconds, so nothing is lost if something interrupts.
Saved visits
Every visit is saved. Find past ones under Past consultations; open one to review it (read-only) and copy its note.
Managing users (admins only)
If you signed in with the shared access code, a Manage users button appears — add a doctor (email, name, password), remove one, or reset a password.
